- On an endless rope haulage road at a point where there was a considerable dip outbye a tub tan amain and caught deceased. The tubs are attached to the rope by means of a jockey at the side
and the lad who was in charge of the tub before it ran amain had missed getting the rope into the jockey but the weight of the rope lying on the tub carried it forward to a steeper part of the road when it ran and as for a considerable distance in front of it, and beyond the point where deceased was, there were no other tubs on the rope it gained considerable speed. The Local Inspectors reported 'It has been a pure accident although we suggest that a runaway switch be worked by a lever from the bankhead be placed also that a refuge hole be made on the right hand side of ropeway.'.
