He had apparently been travelling up the main haulage brow while the haulage was in motion contrary to orders as set out on several notice boards. When about 24 yards from the brow top a lasher-on heard a shout and at once stopped the rope and deceased was found under two full tubs. His cap had come up to the brow top on a full tub shortly before, which rather suggested he had been riding on the tubs and was knocked off, and in getting on to his feet again was knocked down and run over by the succeeding full tubs.
