- Deceased was hewing in a lift 5 yards wide in the broken workings in the Shield Row seam when a stone from the roof 6 feet by 2 feet and 2 feet thick fell from the roof upon him. A depression in the roof had just been passed which extended half across the place and it was part of this depression next the face that fell relieved by a jack filled with clay and a coaly parting above. The place had been examined by the deputy before deceased started work but not during the course of his shift which however had not terminated. Deceased appears to have delayed setting timber of which there was a sufficient supply as prescribed by rule and rendered necessary owing to more roof being exposed by working off the coal after the deputy's visit. He was found under the stone by the undermanager. Local Inspectors reported ‘we think it was a pure accident there being plenty of timber set and sufficient lying at hand.'
