Inspection made. Deceased was a hooker-on at the Orrell five feet mouthing when the plank floor upon which he stood gave way just at his feet and he was fatally injured by falling into the dib hole down by the side of the cage hole a distance of twenty yards. The plank had not been fastened properly after putting dirt down the shaft and the persons responsible were severely censured at the inquiry for their carelessness.
