Details of Event
An incline rope broke and allowed a train of six full tubs to run amain. Goodwin, who was waiting at the bottom of the incline, was run over and fatally injured. The rope had previously broken close to the same place and had been joined by means of a reef knot. A new rope should have been provided after the first break.
