Fell into a hole 5.5ft deep in a recess by the side of engine plane in which the haulage tightening sheave worked having hurriedly entered the place to keep clear of passing journey. The rail which fenced the recess had been removed. There was a proper manhole 6ft away. Died in 5 days.
