Details of Event
Deceased and six others were ascending the upcast shaft upon the top deck of the cage. Deceased was sitting down at the end of the cage, and as the cage neared the bank he probably commenced to get up ready for getting out. As the cage lifted the doors which close the shaft top, the rush of air caused him to lose his balance and he fell; his head was jammed between the cage floor and the bearer supporting the flat sheets. He was instantly killed. Fence gates are provided, but when not in use slide upon bearers at the top of the cage. On this occasion the onsetter had failed to place the gates in position.
