At a junction in the downbrow a timber tub and two empty tubs were standing to be drawn up the brow, the attendant saw deceased standing at the top corner of a manhole just above the tubs, signalled to go on; deceased stood at the top corner whilst the tubs came off the rails and crushed him against the top corner of manhole. He was travelling outbye.
