- He, with two others, was engaged widening a shaft. He was standing on a ledge of ground on the hanging wall, 6ft. above a fixed scaffold, filling the skip with debris. The ledge of ground fell away and precipitated him and the scaffold into the water below. After the fall it was seen that a slip or "soapy head" had formed the back of this ground. If he had worn the life line which was provided he would probably have been saved.
