Details of Event
He was about to take out a girder on a crossgate and was standing near when a fall of roof occurred bringing down the girder and fatally injuring him. The girder was supported by two props and the girder end had sunk into one of them. Two weight bumps occurred about 10 minutes before the accident which was caused by gradual subsidence and insufficient support to one end of the girder. The specified distance for roof supports was not exceeded and the place had been inspected by a deputy 2.5 hours before and reported safe.
