- Under the deceased's directions a cab of a tank locomotive was being placed in position on the locomotive after it had been removed for repairs. To lift the cab a chain from an overhead crane had been passed under the top of the cab, and in order to prevent the cab top buckling when the weight came on the chain, a batten 7 inches by 2.5 inches by 7 feet 6 inches long had been fitted under the top of the cab, between the side angle irons, and under which the chain passed. The cab was placed in position by the crane and the chain removed, but through an oversight the batten was left up, it being held to some extent by the angle irons of the cab. As the men were trying to fit the cab exactly into position deceased came up, and whilst standing on the ground he put his head inside the cab to see how to fit a bolt-hole, when the batten, probably disturbed by the movements of the cab, fell down striking his head, causing him to fall hack and to again strike his head on the edge of a balk of timber lying on the ground. He never regained consciousness and died the same afternoon.
