Deceased was taking loaded tubs as delivered on quarry bank by an endless rope, and conveying them to the dirt tip. Owing probably to an obstruction of the loads at some part of the road the rope momentarily slackened and dropped off a horizontal wheel, about 250 yards from where deceased was at work. It then tightened round the vertical shaft, jammed on the pedestal and ultimately broke near deceased, and between him and the haulage engine. The back lash of the loose end struck him on the side, causing injuries which terminated fatally two days later.
