- Deceased and another pikeman working together in a heading in the Seven Feet Seam. A shot-hole had been charged with 0.75lb of compressed powder and the fuse lit, but it missed fire. After waiting two hours deceased returned and commenced to unram with a punch, with the result that the charge exploded and he was severely burned and bruised, dying from his injuries on the next day. Deceased had been told by the deputy not to go back into the place and his mate also warned him not to do so, but his reply was that he was anxious to get some coal out as they had not been doing well. In unramming the shot he committed a breach of General Rule 12.
