- Deceased and another were filling chalk waggons from a shoot in the roof of a heading 9 t. 3ins. wide, driven into the quarry face, the heading being lighted by three ‘flares’ or hand lamps. He was clearing away seine chalk which had fallen from the first waggon, when the other man after giving a shout of warning brought forward a second one at a slow speed. Deceased appeared either to have been getting into a manhole or sprag-hole at the side of the heading, or, being in it, to have incautiously put out his head to see the approaching waggon. He was caught and crushed between it and the side of the heading, and died in a few minutes. The sprag-hole was only 2ft. ins. wide by 2ft deep, and the clear space between the side of the waggon and that of the heading was only 4ins. There appeared to be no reason why the sprag-holes might not have been much larger.
