Deceased and three others were engaged on a bottom canch, 3.5 feet thick, in a leading place about 4 yards wide driven in advance of a longwall face fitted with conveyers in the Second North District of the High Main seam, 3.5ft thick, of No.2 Pit. After taking up the canch they were putting in 12 feet permanent balks. Two shots had been fired but part of the canch was left on at the right side and a third shot was to he fired. Deceased was filling stones into a tab when a stone fell from the roof and seriously injured him and he died 3 hours later The stone was 6.5 feet by 4.5 feet and about 11 inches thick at one side tailing out to a sharp edge at the other, and with a slip at the end next the face. Its upper surface was smooth and glassy in its fall it displaced a prop and plank. There was plenty of loose timber ready for use. The deputy had inspected the place before work was started and a chargeman had visited the place just before the accident. The Local Inspectors in their report simply stated they had visited the place. Deceased was not a member of the Miners' Union.
