Deceased, usually employed as a driver was on the day of the accident and for the previous day employed greasing the rollers on two self-acting inclines having been warned on the first day both by the undermanager and rolleyway man to keep clear of the sets. He left the bottom of the second incline which inclines three inches per yard and runs with 10 tubs in each set about 3 minutes before tubs were run and passed out of view under three canvass doors near the bottom. The sets appeared to meet with some obstruction near meetings and the lad from the bottom went up and found deceased under the first and second tubs of the full set which were derailed about 60 yards from the bottom of the incline and just below meetings. His cap and grease box were found 8 or 9 yards higher up. Refuge holes were provided as required by law and there was also sufficient room to stand at the sides clear of the tubs. The Local Inspectors reported that in their opinion it was a pure accident.
