Crushed by machinery. Leaning over the cog wheels to push the fly wheel, crank being on the centre. Having turned steam on to the engine when the wheel moved he overbalanced and fell, his right arm being crushed cog wheels. He had been told by foreman an hour before not to put steam on the engine before push crank over the centre.
