The deceased was crushed beneath a loaded hutch as a rake was being hauled up a dook. He had been in a refuge hole and had apparently tried to jump on to the rake in between two of the hutches. The signal wires had not been extended to the end of the haulage road as they ought to have done prior to its being put into use.
