His clothes became entangled in the end of a shaft connected with the screening machinery on the surface, which was not fenced, and which being only 8 or 9 inches in length, if it were not for a set screw in the collar on the shaft to prevent a clutch getting off, would not have caused this accident. The omission to fence was an error of judgement.
