He was helping to dismantle a small hauling engine in the fitting shop. One half of a cog-wheel was suspended by means of ropes and pulley blocks attached to a horizontal beam resting upon two roof principals Deceased was using a pinch bar to force the half-wheel over the engine frame, when, owing to the diagonal strain put upon it the beam slid off its support at one end and fell striking him on the head, killing him instantaneously. The beam should have beam lashed to its supports.
