- Deceased was working at the face of a winning level 7ft. wide, in Bowling Alley seam. At the dip side he had cut in about 8ft and 2.5ft. wide; he then holed the rise coal 2ft. under and spragged it, and, to get farther under, was wedging down a piece of hard coal at the front of the holing, when the overhanging coal suddenly fell and crushed him against the dip side. Deceased had holed into a slip, which was exposed after the fall. His injuries resulted fatal four days afterwards. Died on the 4th.
