- He was drilling a shot-hole at the face at a point which had been undercut to a depth of 2 feet 6 inches, when a piece of bind, 10 feet by 2 feet by 10 inches, fell upon him, fatally injuring him. The face at this point was about 40 feet deep, and it was not being worked in accordance with special Rule 1. The quarry had not been examined that day.
