Caught by runaway empty tubs when travelling up haulage brow, the tubs ran away because the rope got fast hold in the clip and the clip on reaching the automatic knock off fastened causing such a strain that the rope drew at a splicing. The full tubs were held by trailing drags, the empties ran and caught deceased as he was getting into a manhole.
