- Deceased was killed instantly by a fall of top coal in a breasting level in the Cockshead seam. The level was driven 13 yards wide and supported on packs, leaving a roadway about 12ft. wide in the centre. The bottom coal only was got, 4ft. of top coal being left in the packs and ripped in the roadway as required to make height. The tops were up for a distance of 20 yards from the face, and Findlay was in the roadway coupling up a journey of loads, when a goth of 12 yards, burying him beneath it, occurred, and, without warning, the coal fell in the road over a length of 12 yds burying him beneath it.
