- While a set of 30 tubs was gravitating inbye on a dip engine plane and drawing the main rope after them 18 tubs became detached and ran amain, fatally injuring deceased who was near the rapper lever in a landing at the bottom of a drop staple. The tubs are coupled by a chain fixed on one tub fitting into a double eye on the next and secured by an iron pin attached to the tub by a small chain. The pin appears to have come out. Nothing about the coupling was broken. Deceased was near a refuge hole but the noise from other ropes running near may have prevented him hearing the runaway tubs until they caught him. The Local Inspectors reported they were 'of opinion that it was a pure accident.'.
