Deceased was attending to the endless rope haulage, where the tubs are coupled to the haulage rope by clips on lashing chains. Owing to a clip coming loose on the rope a full tub ran amain on a gradient of 1 in 9 and caught the deceased before he was able to enter a refuge bole. The refuge holes were 9 yards apart on this portion of the haulage road. He died on December 17th from the effects of the injuries received.
