- Deceased was injured by a fall of roof in the Hardmine Seam while making room for a face prop near the buttock. He died from the injuries five days later. The fall came from three slips and a break. One slip which was perpendicular to the face was a small dislocation with a throw of about 2ins. and was easily visible before the fall occurred. Deceased in view of the presence of the visible fault slip should have put up a temporary middle or foreset prop before continuing the removal of coal to make room for a prop at the face. Such a prop would almost certainly have prevented the fall which only extended in all over an area of 4ft. 6ins by 5ft. and attained a thickness of 20 ins. against the fault slip.
