Inspection made and inquest attended. The front full tub of the journey ten tubs on a self-acting incline got off the rails and got fast against a leg near the bottom of incline where it was only a single road and of small area. An attempt was made by the fireman to sprag up third tub from the bottom then asked deceased to liberate first tub and when he got it free the sprag gave way and the three tubs got unshackled and crushed the deceased before he could clear.
