Inspection made and inquest attended. A piece of coal feel don the shaft a distance of 500 yards when pulling out the full tubs of the second deck at the surface. It must have fallen through the space of six inches clearance allowed for the cage at pit bottom striking him on the head. He succumbed to the injuries on the 24th.
