Deceased was riding on the chain at the front of a journey of three loads in an engine dip inclined at 1 in 2.5 in the Redmine seam. He fell off in front of the tubs and was run over receiving injuries from which he died two days later. Deceased stated that one of the loads caught the roof and he was jerked from his position.
