- Deceased attached the main rope to a journey of 24 trams of coal, and signalled to the engineman to pull out. As the journey started the pin of the second shackle broke. Owing to the contraction of the rope the two trams jerked rapidly forward and struck deceased violently as he was crossing the road. He sustained a compound fracture of the right leg which had to be amputated a week after gangrene set in, and he died on the 17th day after the accident. The broken shackle pin could not be found.
