Deceased came out of the level at the bottom of the endless rope haulage downbrow to take an empty tub from the set standing in the brow. This set of 13 tubs had been previously secured by means of a chain provided for the purpose, but later they were intermediately uncoupled by some repairers engaged higher up the brow. Deceased was attempting to place the lowest down empty tub on the rails when some of the other tubs moved away and crushed him against the low side of the level.
