At the bottom of the shaft the cage is unloaded and loaded simultaneously at an upper and lower landing, cage descending with 12 men in the bottom deck and 10 men in the top deck, before the cage landed deceased raised the fence gate at the tipper landing, and as the bottom of the cage approached put out his hand to raise the cage gate, he was caught by the middle hoop of the cage and killed. It was a fatal mistake taking the bottom deck to be the top deck, he ought to have waited until the cage was at rest. The men in the bottom deck carried lighted lamps, the height of the bottom deck is three feet greater than the top.
