A cage containing four tubs, two on each deck, had just been brought up the shaft and he had unloaded the bottom deck and put two empty tubs into it. He then signalled to the winding engineman to lower the top deck on to the fallers. The engineman lowered the cage but the deceased had not got the fallers in and the cage went past the level of the landing. He was attempting as the cage was being lowered to lift the catch which secures the tubs in the cage and was carried with it causing him to fall with his head and shoulders on the top of the tubs and he was crushed between the top of the cage and the floor of the pit bank and instantly killed.
