Deceased had attached an empty tub to the chain, at the bottom of a jig 40 yards long and having a gradient of 1 in 3.5, signalled, and was waiting while the journey was being run, when the chain broke and the full tub crashed into him and killed him instantly. The breakage was at a patent malleable iron coupling link used for lengthening the chain. Steel couplings have been used since this accident. Deceased ought to have stood aside in a safe place while the journey was being run.
