Details of Event
Deceased, when lowering two empty trucks towards the screens, was sitting on the handle of the brake lever at the front of the first truck his lags hanging over the rail. He slipped off the lever and fell with his leg across the rails and the front wheel passed over it. He died from shock four hours later. A brake stick was supplied but it was alleged that it could not be used in this instance as owing to the brake block being worn the lever was too far down.
