- Deceased was holing in the bottom coal, and bad undercut about 10 feet long and 3 feet 6 inches deep. He had two short sprags set under the coal, and a "head sprag" set to the middle of the seam. While he was lying on his side holing, a large piece of coal, about 8 feet long and 1 foot 6 inches thick, fell over on top of his head and shoulders, causing instant death. The coal parted away from a facing, and displaced one of the abort sprags and the head sprag.
