He was run over by three of seven trams which had run away down a steep self-acting incline, owing to the failure of a coupling hook. There was no evidence of unusual strain on the coupling. Two persons stated that they had examined the coupling a few minutes before the accident happened and considered it safe, although it had been running some time and was worn. Proper refuge holes were provided at the bottom of the incline, but deceased had so placed a train of empties that they blocked the entrance to one of them. His body was found under the tubs opposite to the next outbye refuge hole.
