Inspection made. He was guiding the tubs round a curve on the haulage road and had to loosen the chain attaching the tub to rope as the tub approached the curve pulley and when doing this appears to have had some difficulty and seems to have persevered until his arm got fast and his head got crushed against cross bar fatally injuring him.
