A lift in the broken workings of Main Coal seam had holed into the old bord. Preparatory to holing a prop and plank had been set by deceased’s marrow close to the coalface and pair of gears was set 5.5 feet further out than the plank end. Shortly after the deceased started work in the back shift a mass of stone weighing from 1.5 to 2 tons fell on him from between the prop and plank and pair of gears and he was found by the back overman covered up and quite dead. The stone fell from between two jacks and two slips a had a keen parting above. A thin skin of coal adhering to the roof stone made it difficult to observe jacks slips. The deputy was in the place during the previous shift and about 2 hours before the accident when loose coal prevented him putting up a pair of gears near the face which was then only 3 feet in advance of the pair already set. There was no lack loose timber. The Local Inspectors reported ‘we believe that there is not any blame attached to anyone for accident.’
