- On Surface – By Machinery – He was starting the screening plant engine which had centred. After opening the throttle valve he was turning the fly wheel by hand, and when the engine got off the centre it ran faster than he expected. He was pulled forward and pitched down through a hole in the floor on to the sidings, 12 ft below, alighting on his head, and sustaining a fracture of the skull and a fracture of the right arm. He never regained consciousness and died about 12 hours later.
