Inspection made and inquest attended. When the journey of nineteen full tubs had started about twenty yards from bottom of downbrow two tubs got off the rails and the haulage rope broke away from the capping and the tubs ran back crushing the deceased who was in the act of signalling for the gang to stop. The drag had not been attached at the starting place but was to be attached at the gateway end where accident occurred. It was recommended that the drag should be attached previous to the journey starting from bottom of downbrow.
