He had just finished a cut with a Hurd Bar Coal Cutter, and the bar bad been racked from under the cut by his mate. Unfortunately they neglected to stop the machine. Deceased went to the jack to lift the bar so that they could get the picks out, and in some way fell over backwards on to the revolving bar which dragged him under, inflicting fatal injuries.
