- Deceased was employed pulling off the full tubs at the bottom of a staple. After he had rapped the cage with the empty tubs away, the descending cage with the full tubs caught against the staple side about 2 feet down, the tub being off the rails. While the lads at the top were lifting the tub into the cage, levering it with a 5 feet 6 inches long prop, deceased started to clean the bottom of the staple up with a shovel, leaning forward into the shaft, having pushed the fence gates on one side. The lads at the top let the prop fall down the staple which hit deceased on the back of the head. He died the next morning.
