Run over by tubs in engine dip through the breakage of a coupling chain allowing nine tubs to run back. He had been repairing a rope in the dip and the journey was started before he was out of the dip. The coupler which broke was of a type which had been discarded for use in the engine dip but continued for use on the levels. By some mistake or carelessness it was used in the dip on this occasion. It was recommended that all the old weak couplers be brought out of the pit.
