After signalling "three" to the winding engineman, and when the latter was lowering the cage consisting of four decks to the mouthing, deceased, who was about to ascend the shaft, got struck on the head, and fatally injured by the cage. It is supposed that deceased was loosening the chain which acts as a guard on the bottom deck, when the cage was moving.
