- Deceased was holing at the top of the coal in a drift in the Ten Feet seam. He holed to the junction of two slips which converged horizontally and vertically. One of these had been observed and timber set accordingly the other was not observed and ran parallel to and inside the next row of posts, distant about 5 feet from the face row. The roof between the slips fell over a length of about 8 feet and a width of 3 feet. Deceased was buried by the fall and killed instantly.
