- Deceased went up jig (inclination 8 degrees) about half way, a distance of 14 yards to lift a derailed empty tub on to the rails. He called to the jigger to give him some slack rope. The latter put his foot on the rope with the loaded tub on it and gave a jerk, when the shank attached to the jaws of the jig pulley broke inside the jig post where the weld of the shank to the jaws had been made. The fracture appeared to be quite new and the weld to have been good. The jig wheel and jaws had been in use for at least six months but on further examination than the ordinary daily inspection by the fireman of the district.
