- Deceased and several other hand putters two of whom were seriously injured were riding inbye contrary to the regulations in the set of 6 empty tubs drawn by a pony blind in the right eye on a rolleyway extending from the bottom of a staple in the Brockwell seam. The road was 12 feet wide and 6 feet high and nearly level. In making the rolleyway about 1 foot of roof stone had been taken down and about 1.5 foot of sagre below the seam taken up. The road was timbered with balks 9 feet long and 9 inches by 4.5 inches set about 6 feet apart supported by props at each end. The roof where the fall took place is grey metal. The driver was in the first tub with a putter and he got out to change some switches without stopping the pony the tub got off the way and drew some of the props on the right side and a heavy fall of roof took place. A slip extended across the road in a diagonal direction from which the stone fell. The fall was 21 feet long by 10 feet wide and up to 18 inches thick. Deceased appears to have been attempting to get out of the tub when he was caught.
