Fall of roof on engine dip. He was sitting to take an electric motor down the dip and thinking it would not pass under a certain bar he began to cut away the underside of the bar with an axe. At the second blow the bar broke causing a fall which killed him. The bar was of larch, 9 to 10 inches in diameter and appeared sound but was not so at place where it broke.
